Reablement support
Our Reablement North team is here to help you regain or maintain your independence at home, following illness, injury, or a stay in hospital. We provide short-term support to help you build confidence and carry out daily activities safely.
What we do
Reablement support can last from a few days up to a maximum of six weeks. The aim is to help you recover and manage as independently as possible in your own home.
We can support with:
- personal care
- preparing meals
- building confidence after a period of illness or reduced mobility
- physical and emotional support with daily living tasks
At the start, reablement is free of charge. However, following a full assessment, you may be asked to contribute to the cost of ongoing support. A financial assessment will be completed to determine this.
If specialist dementia reablement is needed, the service may be supported by an Admiral Nurse, and where appropriate, other professionals such as Occupational Therapists or Physiotherapists.
We will assess whether any equipment is required to support you safely at home. If a lifeline alarm is installed and you do not have a nominated responder, our team can act as a named responder between 7am and 10pm.
Referrals
If this support is required you will be referred to the Reablement team by a healthcare professional.
