Community occupational therapy

The Community Therapy Service aim to support you to live your best life with your disability or long-term condition and connect you to your local community.

The service provides a free assessment, support and advice for adults. This can range from the loan of equipment, arranging minor adaptations, recommending major adaptations or an onward referral to another specialist service.

We also provide assessments for children who may benefit from adaptations to the home. 

Assessment

During your assessment, information is gathered about what matters most to you, your strengths (what you can do), information about your disability or condition, and the difficulties or challenges you could be having.

This is a two-way open discussion covering all areas of your life which are being affected.

Recommendations may include:

  • equipment - such as perching stool, bed and chair raises and raised toilet seat
  • minor adaptations - such as grab rails, stair rails
  • major adaptations - such as stairlifts, wet rooms, ramps
  • housing - explore housing solutions for those people who need to move house
  • specialist moving and handling - such as recommendations for equipment, handling techniques and training for informal carers and care providers in complex situations
  • community reablement - advise on new methods of carrying out daily living activities and practice sessions
  • Thackley Green Specialist Care centre - short-term care to those discharged from hospital or admitted from the community to enable them to recover their independence

Complete the below referral form to request an assessment by the Community Occupational Therapy team. 

Referrals can be made by yourself, family, carers, health and social care professionals.

You can also call 0300 126 3000, and select Option 1, Option 2 and Option 2 to complete a referral.
