Activity on Referral scheme

Activity on Referral scheme helps people to increase their levels of physical activity in a safe and structured environment. The service provides health screening, advice and access to exercise, led by highly qualified exercise professionals.

Eligibility 

People who are aged 16+years of age have a long-term illness or chronic medical condition, such as:

  • diabetes
  • high cholesterol
  • high blood pressure
  • joint
  • muscle and mobility conditions
  • mental health problems
  • pulmonary or cardiovascular disease
  • body mass index (BMI) above 30 or 28 with co-morbidities and you are currently inactive

Apply

Talk to your GP, nurse or healthcare professional about how being more active would help you. If they feel it is appropriate, you may be referred to discounted exercise and activity sessions. Any health professional such as your GP, practice nurse or social prescribing link worker can refer you on to the scheme.
