IMR Recommendations
Kettering General Hospital (KGH)
- KGH to undertake a safeguarding audit to ensure all safeguarding referrals are recorded in a way that allows confirmation of submission, receipt and outcome.
- Staff members to be aware of historical noting so appropriate conversations can take place during each admission to the department.
Northamptonshire Healthcare NHS Foundation Trust (NHFT)
- NHFT to ensure the recording of visitor information when visiting inpatient on either sectioned or informal basis.
- NHFT to ensure greater liaison between carers and family members to enable appropriate and safe discharge and this should be considered moving forward by the ward (St Mary’s). This will aid improving the service user and carer journey.
- NHFT to individualise care for patients above and beyond the signposting to the IRH and crisis café and telephone contact to include the patient and their support network.
- NHFT to improve overall communication with their patients about their services and ensuring that there is suitable safety planning whilst patients are waiting for services.
- NHFT to provide staff training on routine enquiry and embed into practice.
- NHFT to ensure all staff taking telephone calls from service user carers/family should be checking names and relationships to ensure that documentation is accurate.
- NHFT to raise the subject of domestic abuse during risk assessments.
- NHFT to ensure risk assessments are thoroughly documented in line with Trust policy and are completed comprehensively and appropriately to enable suitable review of risk.
- NHFT to liaise with CGL to be part of the Mental Health discharge pathway.
- NHFT to improve communication between multidisciplinary teams and partner agencies about patients’ current needs and their care plan to ensure cohesive care and support is provided.
Northamptonshire Police
- Police to improve the quality of PPN’s to include wider context of Police attendance to incident.
Northamptonshire ICB/Primary Care - Provider of GP services
- Primary Care - A “think family” approach to be taken by the practice around risks to the children in the family where there is a vulnerable adult who is experiencing mental health issues.
- Practice to consider a stand-alone Domestic Abuse policy that covers both patients and staff members. Policy to include a pathway detailing the process for responding to domestic abuse in the practice.
- Awareness raising around men being a victim of DA as they are often overlooked.
Northamptonshire Children’s Trust (NCT)
- NCT to ensure awareness across partner agencies instructing them to be explicit in MASH referrals about any identified risks or concerns relating to domestic abuse in families where there are children.
East Midlands Ambulance Service (EMAS)
- EMAS to implement a drug and alcohol pathway.
Change Grow Live (CGL) / S2S
- CGL to liaise with NHFT to be part of the Mental Health discharge pathway.
- Improved information sharing with partners agencies. S2S to attend weekly mental health multi-disciplinary meetings.
