Reablement Support Workers

We have a reablement service across the North of the county, where we provide a range of support for our customers in their homes.

The post holder will work with customers developing, implementing and updating support plans, in consultation with other interested parties, which support independence and help minimise the risk of hospitalisation or residential care. You will also respond to customers in a crisis and provide personal care as required.

A full driving licence and access to a vehicle is essential for this role.
