Shared Lives referral process

1. Referral
Referrals are given to our Brokerage team who forward them onto our Shared Lives team.
2. Assessment
We will arrange an assessment visit if you have an eligible need for adult social care (identified under the Care Act).
We aim to book this within 7 to 10 days of receiving your referral. A visit will usually take 1 to 2 hours and will look at your:
- needs
- wishes
- preferences
- hobbies
- interests
- abilities
- disabilities
3. Review
We will reviewed your assessment within 1 to 2 days and then look at potential suitable carers to support you (whether it be for a long-term or a short-term placement).
4. Matching
The assessment will be forwarded to any potential carers to review.
They will decide if the placement is suitable for their family environment and community, as well as being able to meet your needs.
We ask the carer to make a decision within 7 days.
5. Introduction visit
An introduction visit between you, the carer and anyone else involved in the care will be arranged.
This gives both you and the carer an opportunity to decide if the placement would be suitable for both parties.
We aim to book the introduction visit within 1 to 2 weeks after the carer has reviewed your assessment.
Further visits (such as a tea visit and an overnight stay) will be arranged giving more opportunities to agree to the placement.
These visits are discussed and planned between you and the Shared Lives carer.
For a long-term placement, a move-in date will be agreed and you would then become part of the Shared Lives scheme.
Initial visits between you and the carer can take place as many times as needed and over time - as everyone feels necessary.
6. Review period
Once the placement has started, we will review the placement with monthly visits over a 6 month period.
We can see if the placement is working for you or not, and if an alternative placement is needed.
Once the initial 6 month period is over, we will carry out 6 monthly reviews with you.
